Job Summary


The Patient/Resident Care Assistant/Home Care Aide provides direct care to patients/ residents/clients in need of assistance or total care and activities of daily living.  Through written and verbal communication, essential information is shared with health care team members.  Completes duties as assigned by the Registered Nurse or designated healthcare professional (HCP) in accordance with federal, state, and local regulations and within the polices, procedures, guidelines, and department-specific addendum of the Mercy Health Partners.



  1. Contributes to the database of patients/residents/clients under direction of an RN/LPN.
  2. Documents patients/residents/clients data according to policy.


  1. Communicates effectively in a timely manner with care team members regarding patients’/residents’/clients’ and unit/area needs. 


  1. Performs procedures as defined by unit/area specific standards.
  2. Demonstrates ability to organize prioritize and adapt to changing needs of patients/residents/clients and unit/department/facility.
  3.  Provides a safe and supportive environment for all patients/residents/                      clients by demonstrating an understanding of:
  • patient/resident/client rights,
  • confidentiality,
  • abuse and appropriate interventions,
  • Advance Directives;

      4.  Assists in identification of unit and client needs, ie., call lights.


  1. Contributes to the evaluation of the patient’s/resident’s/client’s response to nursing interventions through communication.


  1. Attends and participates in unit/department meetings per unit policy.
  2. Attends required programs.
  3. Actively participates in performance improvement


  • High School Graduate or GED required.
  • CPR Proficiency
  • Recent healthcare experience preferred
  • Computer experience preferred

Hours:  Full-Time, 36 hours per week, 7:00p - 7:30a, every third weekend
