Job Summary

Mercy Health – Clermont Hospital is opening a newly renovated 7,400 sq ft Senior Behavioral Health Services Inpatient Geriatric Psychiatric Unit. The unit will will enhance Mercy Health’s ability to fulfill our mission by providing patients with rapid access to high-quality senior behavioral health services in a warm, supportive atmosphere. Senior Behavioral Health Services is an acute, short-term program for patients aged 55 years and older in need of a secure, therapeutic environment 24-hours a day.

 The first thing our professional team will do is a very thorough evaluation to help determine what’s causing the behavioral and mental changes. We also check for any underlying medical or biochemical origins. We will then work with the patient, family and other health professionals, to develop a course of treatment. Treatment includes individual meetings with psychiatrists; group therapy with seniors struggling with similar problems; and educational programs. Our nurses, psychiatrists, trained counselors and social workers can help with all aspects of a person’s life.

JOB SUMMARY:

The Social Worker/Counselor II leads the planning and coordination of patient care.  Responsible for providing social services for patients and families of inpatient behavioral health as part of a multidisciplinary team with emphasis on group and individual therapy.  Works closely with psychiatrists, as an active member of the Treatment Team by attending Care Plan meetings.  Responsible for psychotherapy group, wellness groups, psychosocial assessments and faciliting individual, marital and/or family therapy. Develops, implements, reviews and updates treatment plans as well as case management and discharge planning. 
Ensures assessments are completed within regulatory timeframe. Has ability to perform all requirements of the social worker/ counselor I.  Leads weekly clinical supervision meetings as assigned. Designs and implements specialized therapeutic groups for patients and families in all program areas, including education, support services and community resources. 

MINIMUM EDUCATION / QUALIFICATIONS / LICENSURE / CERTIFICATIONS:

  • Graduate Degree in Social Work, Counseling; required
  • LISW
  • Certified Group Therapist within 1 year of hire
  • 2-5 years' expierience working with individuals with co-occurring mental health disorders and substance diagnosis.
  • Experience in working in a psychiatric setting highly preferred.
  • Basic Life Support certification required within 30 days of hire.
  • De-Escalation training within 90 days of hire; Group Psychotherapy Training.
