Job Summary

Sign on Bonus. Competitive Wage and Shift Differential.

The LPN renders direct and indirect nursing care to the residents using the nursing process. Oversees other nursing personnel on the unit in accordance with facility mission and values. Implements professional nursing standards on the unit. Under the direction of the RN Team Leader and the RN Supervisor, the LPN plans, implements and evaluates health care according to established care standards. The LPN is responsible for assigning daily tasks to the STNAs and assists the RN Team Leader with daily operations of the unit. The LPN reports directly to the RN Team Leader or the RN Nurse Supervisor.

Qualifications - Minimum

Education:

ESSENTIAL JOB ACCOUNTABILITIES:

1.   Assesses the resident during shift and monitors care rendered by all staff members.

Responsible for completion of necessary documentation on the resident.  This includes but is not limited to:   Monthly summaries, post-admission charting, PIVs, PAVs, monitoring of vital signs for any other reason, LOAs and returns, transfers, etc.

Makes frequent (at least 2 per hour) and purposeful rounds during shift to monitor residents’ condition, oversee and delivers care to meet resident’s needs.         

Notifies the Unit Manager or physician of any pertinent resident findings.

Makes resident care assignments for STNAs and delivers care as needed.

Evaluates the care given and addresses areas of concern with the Unit Manager.

2.   Initiates and follows through, directly or indirectly, with appropriate nursing action or care delivery to meet residents’ needs.

Ensures that the resident’s rights are respectfully upheld.

Maintains a safe environment for residents and employees.

Secures appropriate equipment and administers or assists with procedures or examinations.

Administers medications accurately and safely, following facility policy and procedure.

Administers treatments accurately and safely following facility policy and procedure.

Administers resident care as ordered according to facility policy and procedures.

Adheres to policies and procedures regarding infection control.

3.   Assumes a leadership role in providing resident-focused care by planning, coordinating care and by setting priorities.

Functions as a Charge Nurse

            1.         Make resident care assignments

            2.         Give/receive shift to shift report

            3.         Reports resident condition changes to RN Team Leader and/or RN Supervisor by the end of the shift.

Accepts leader/charge duties as assigned.

Possesses initiative, judgment and resourcefulness in emergency situations.

Participates in staff meetings, unit meetings/activities and departmental committees or sub-committees.

Participates annual performance appraisals and proficiency evaluations for assigned STNAs.

4.   Assumes responsibility for job performance and for ensuring continued professional growth.

Delivers resident care based on standards of nursing practice and in accordance with facility policies and procedures.

Demonstrates support of quality improvement efforts through participation in unit-based activities.

Attends in-service and continuing education programs as evidenced by documentation.

Complies with Ohio Board of Nursing requirements to maintain licensure.

SPECIFIC DUTIES:

 1.         Med administration for assigned residents.

 2.         Perform treatment as ordered by physician on assigned residents.

 3.         Making rounds on residents during the shift (at least every 2 hours) checking on all residents who are ill/unstable, have sustained an injury or experienced anything eventful in the past 24 hours.

 4.         Receive and place physician calls - take orders - transcribe orders.

 5.         Responsible for informing physicians of any lab results (critical values or abnormal for which the resident is symptomatic) and x-ray results.

 6.         Completion of monthly summaries.

 7.         Responsible for filling out transfer sheets and other related documentation.

 8.         Schedule outside appointments and transportation.  Completes “Outside appointment checklist”.

 9.         Give and receive shift to shift report.

 10.       Provide continuity of care on a daily basis.

 11.       Direct STNAs and give them needed information or changes for resident care that day.  This may include carrying-out disciplinary action on an employee who fails to perform satisfactorily.

 12.       Report problem areas, care issues and changes in condition to the RN Team Leader.

 13.       Initiate/complete Acute and Transient Sheets.

 14.       Communicate with Dietary any changes in resident diet orders or food preferences.

 15.       Chart/document in Nurses Notes, MAR or TAR as required.

 16.       In the absence of the RN Team Leader or if directed, the LPN may be required to:

             1.   Perform/complete admission procedure.

             2.   Make rounds with physicians.

             3.   Notify physician or acute or significant changes in resident condition.

 17.       Uphold and abide by facility policy for all nursing procedures.

 18.       Direct, assign, assists, oversee and evaluate STNAs on unit for respective shift. Provides direct nursing care, beds/baths, ROM, feeding, etc., as needed on the unit.

19.       Respond to family members or visitors in a professional manner. Listen to any concerns or complaints they may have. Attempt to resolve problems, include Unit Manger or supervisor if necessary. Follows up and reports back to family in a timely manner.

20.       Attend monthly General Nurses’ Meeting.

21.       Report any repair or maintenance problems on unit (fill-out requisition) to the Director of Plant Operations.

22.       Maintain Med Room and Charting Room in clean/orderly fashion.

23.       Weekly cleaning (or more frequently) of Med carts, Treatment carts and Med Room refrigerators.

24.       Fax orders to MediRx pharmacy on a daily basis.

25.       Checks Nurses notes to ensure complete charting of significant changes and for incident reports.

26.       File labs and x-ray reports in charts.

27.       Notify front desk, social service, secretary, personal laundry and any other staff who need to know about room changes.

28.       Disassemble charts of discharged or expired residents. Include all forms related to documentation/clinical records.

29.       Answers telephone on unit.

30.       Check resident for ID bracelets weekly, and reports to medical records the need to replace any missing ones.

31.       Completes incident reports; notifies family and/or physician of incident. Immediately initiate follow-up report to prevent the incident from happening again with subsequent injury of that resident or other resident.

Hours

Hours per pay period: 56

Hours: 6:30am to 3:00pm

Weekends and holidays are rotated.

Works flex hours days/afternoons.
