Job Description

Job Summary


The mission of Mercy Health is to extend the healing ministry of Jesus by improving the health of our communities with emphasis on people who are poor and under-served.


The Licensed Practical Nurse (LPN) works with his/her team partner(s) to provide direct patient care. As a member of the Patient Care Team, the LPN is trained to perform a wide range of essential patient services including designated medication administration.

The LPN performs a variety of basic age-specific duties to assist in the care of the patients under the direction of a Registered Nurse (RN), Advanced Practice Nurse, Physician Assistant, Dentist, Physician or other legally designated health care provider with prescriptive authority. All clinical care is done under the direction of the licensed health care provider as above who determines appropriate tasks for delegation. The LPN makes observations as directed and reports deviations to the RN and/or provider.

Qualifications - Minimum


High School Diploma/GED

Graduate of an accredited LPN Program

Current license in State of Ohio (Licensed Practical Nurse) with M-IV certification.

Current CPR certification at basic level


Experience in providing direct patient care.  Physician office setting desirable

Computer literacy

Previous experience working in certain specialty areas may be desirable.


Follows general instructions.

Understands several defined work routines.

Reads various references, extracts information, and applies basic arithmetic functions.

May operate complicated equipment/machinery.


1.            Knowledgeable of policies and procedures

2.            Receives patient from the waiting room, escorts to the exam room, prepares patient for exam

3.            Obtains and records vital signs, height and weight, major complaint, medication and allergies in

patient chart

4.            Performs venipuncture for blood collected by following proper specimen, storing and recording


5.            Collects various specimens (i.e. urine, sputum) by following proper specimen collection, storing and recording protocols

6.            Prepares lab specimen in timely manner for transport to hospital lab by maintaining positive

specimen identification throughout processing

7.            Prepares patient, assist physicians with set-up and clean-up of procedures (i.e. pap smears,

biopsies, spinal/bladder taps, etc.) as per protocols

8.            Administers medications and performs IV therapy as governed by licensure, training, and

demonstrate competency

9.            Anticipates patients and other caregivers requests/needs and responds in a timely manner

10.          As per protocol may perform tests or provide treatment as ordered including but not limited to:

a.            EKG

b.            Aerosol Medication Therapy

c.             peak flows

d.            pulse oximetry

e.            ear irrigation

f.             visual screening

g.            EMG prescreening

11.          Performs, reports and records clinical laboratory tests and results (i.e. Strep A, urine pregnancy,

      urine dipsticks, etc.)

12.          Performs and records quality assurance data on equipment according to protocols (i.e. pulse     

      oximeter, refrigerator, emergency cart, pharmacy logs, etc.)

13.          Observes universal precautions and aseptic techniques with patient and the use of medical

       instruments and equipment

14.  Safely operates all medical equipment according to manufacturers’ instructions and

       protocols (i.e. ultrasound machine, EMG machine etc.)

15.  Reports and records patient critical lab values, adverse reactions and pertinent information

       promptly to appropriate provider

16.  Performs computer order entry and documentation in EMR

17.  Able to perform administrative duties (i.e. scheduling appointments, answering phone, filing,

       assembling and filing medical records, etc.)

18.  Provides friendly assistance to patient’s family members and visitors


1.            Performs other job duties as assigned.

2.            Acts as liaison/support person for patient and family members.

3.            Assists with patient education and discharge instruction with RN or provider.

4.            Inventories unit and determines supply needs.  Conveys needs to appropriate personnel.

5.            Assists in daily cleaning of exam rooms and clinic station area.  Assess maintenance needs.
