JOB DESCRIPTION  Title: Community Health Social Worker Department: Mercy Health SelectDate Approved:     Facility:  Home Office                                                                                                Reports to:  Care Coordination ManagerDate Revised:   ___________________________________ _______________________________     (Manager Approval) (Regional Management Staff Approval)  ___________________________________ (Human Resources Approval) CORE VALUES:  It is expected that all of the duties and responsibilities of this position will be performed in a manner that reflects the values of Mercy Health, which are:  Excellence, Human Dignity, Justice, Mercy, Sacredness of Life and Service. BASIC PURPOSE:   This position will provide direct assistance to the Care Coordination Team and patients.  JOBDUTYNUMBER RELATIVEWEIGHT%   JOB DUTIES I. 15% MISSION:Incorporates the Core Values and supports the Mission and Philosophy of Mercy Health. II.   5%    SAFETY:Follows Mercy Health policies and maintains high standards of safe work performance. ATTENDANCE:Maintains a good attendance record as set forth in Mercy Health policies. PUNCTUALITY:Reports to work when the scheduled shift begins.   III.               80%                POSITION:  SPECIFIC ACCOUNTABILITIES • Community based support and assistance to patients and staff to implement care plan as deemed necessary by the Care Coordination Team• Work with independently licensed clinician and physician to assess patients (telephonically, the office setting and patient home setting as appropriate) and make recommendations for social and behavioral interventions as necessary.• Maintain comprehensive progress notes for each contact make.• Participate in regularly scheduled Care Coordination Team meetings• Facilitates patient access to recommended services• Maintains good working relationships with and comprehensive knowledge of applicable community services• Provides basic general health and wellness education/counseling to patients as appropriate• Remain abreast of Community Resources and provide proactive updates to Care Coordination Team• Follow-up on all referrals to ensure customer satisfaction• Identify system barriers in efforts to connect patients to health and social services• Maintains required data and documentation and prepares required report for the Care Coordination Team• Perform duties with minimal supervision.  May be required to occasionally work irregular hours, attend other job related meetings, and perform other duties as assigned.   EDUCATION, SPECIALIZED AND/OR TECHNICAL KNOWLEDGE REQUIREMENTS: • Master’s degree with a Social Worker License• Prefer experience in a mental health related environment working with patients on discharge planning and case management with individuals and families• Prefer experience coordinating responses between civic, religious, governmental and other organizations• Ability to work with individuals, groups and families• Familiarity and knowledge of Community Resources• Preferred knowledge of managed care requirements both public and private• Strong interpersonal skills and ability to work collaboratively with patients, non-clinical staff, clinical staff and project team• Effective oral and written communication skills, communicates information correctly.• Flexibility to work non traditional hours• Works well in a Team Setting.• Personal computer skills.  Experience with database entry, EMR documentation, Power Point preferred• Highly organized and detail oriented.• Accepts responsibility and follows through on projects and activities. JOB REQUIREMENTS:• Physical Requirements• Energetic and able to drive a car with a valid Ohio driver’s license and automobile insurance• Sight and dexterity to operate a PC, copy machine and telephone• Able to speak well and clearly over a phone and at large group presentations.• Valid Driver’s License and proof of auto insurance• Mental Requirements• Remains calm during stressful periods and acts appropriately.• Ability to quickly learn new skills and concepts• Able to adapt to change.• Able to handle multiple priorities at once.• Concentration, reasoning and problem solving skills.• Working Conditions• Office Environment• Hospital and physicians office environment• Patient Home Visits
