Job Summary

MISSION:

SUMMARY OF JOB RESPONSIBILITIES:

The Wellness Coordinator must be a Licensed LPN Nurse with a current Ohio License.  The Wellness Coordinator renders direct and indirect nursing care to the residents using the nursing process.  Oversees LPN and Resident Assistant personnel in assisted living in accordance with facility mission and values.  Implements professional nursing standards in the facility.  Under the direction of the Director, Assisted Living Services and Marketing, the Wellness Coordinator contributes to the planning, implementation, and evaluation of reports directly related to nursing functions.

Qualifications - Minimum

EDUCATION:

Licensed LPN nurse with a current Ohio License.

Valid CPR and First Aide training.

EXPERIENCE:

Assisted Living experience preferred

1-2 years of supervisory experience

Strong clinical and documentation skills

Medication Administration

KNOWLEDGE AND SKILLS:

  • Assumes a leadership role by providing resident-focused care by planning, coordinating care and by setting priorities.
  • Functions as Wellness Coordinator.
  • Make Resident Care Assignments.
  • Give /receive shift to shift reports.
  • Report resident condition changes to physician as necessary.
  • Completes “Level of Care” as required.
  • Accepts charge duties as assigned.

PROBLEM SOLVING / DECISION-MAKING:

  • Monitors Assisted Living care provided.
  • Reviews and documents effectiveness of intervention and implementation of physician’s orders.
  • Reviews and documents level of accomplishment of care goals and expected outcomes following interventions.
  • Monitors delivery and outcomes of care provided by other team members including LPNs and Residents Assistants.  Evaluates the care given by Resident Assistants and addresses areas of concern.  Evaluates non-nursing task functions of LPNs and Residents Assistants.
  • Ensures that the resident’s rights are respectfully upheld.
  • Secures appropriate equipment and administers or assists with procedures or examinations.
  • Adheres to policies and procedures regarding infection control.
  • Assists with referrals as necessary to healthcare providers and other resource agencies.

PRINCIPAL JOB ACCOUNTABILITIES (Essential Functions):

  • Gathers data regarding physical, psychological and social dimensions of the resident.
  • Applies knowledge of illnesses, injuries and diseases and uses available resources.  Consults physician and other health care providers when necessary and appropriate to render safe and effective care to the residents.
  • Documents appropriate and complete information related to resident in accordance with residential care facility (RCF) requirements and good nursing documentation.  Exhibits knowledge and understanding of the regulatory requirements of an Ohio Licensed RCF.
  • Implements care in accordance with physician orders including administration of medication and performance of treatments.
  • Documents nursing interventions, resident response, effectiveness, complications, notifications and other important information.
  • Utilized all available in-house resource personnel and departments for treatment and consultation.
  • Respects the dignity and confidentiality of resident and his/her medical information.  Serves as a resident advocate when necessary.
  • Provides a safe environment for the residents and staff members in accordance with the facility policies and procedures and the residents care plan.
  • Functions within the limits of the Ohio Nurse Practice Act, Humility House and nursing department policies and procedures; utilizes good judgment and provides care and demonstrates skills comparable to peers with similar training and experience, functioning under similar circumstances.
  • Counsel/discipline personnel as requested or as required in compliance with Humility House policies and procedures.
  • Reports resident condition changes to physician as necessary.
  • Plans and implements education for staff as approved by Manager of Performance Improvement.
  • Review and follow-up related to resident falls.
  • Evaluate residents for admission to the Assisted Living facility per physician approval.
  • Possesses initiative, judgment and resourcefulness in emergency situations.
  • Participates in staff meetings, LPN meetings/activities and departmental committees or sub-committees.
  • Provides input and/or participates in annual performance appraisals and proficiency evaluation and competencies for LPNs and Resident Assistants.
  • Complies with Ohio Board of Nursing requirements to maintain licensure.
  • Provides continuity of care on a daily basis.
  • Direct Resident Assistants and provide needed information or changes for resident care that day.
  • Upholds and abides by facility policy for all nursing procedures.
  • Provides monthly resident evaluations to determine levels of care.
  • Coverage of nursing schedule if coverage cannot be obtained in the assisted living facility.
  • On-call as needed.
  • Review documentation, policies for medical care.

JOB ACCOUNTABILITIES – Other:

  • Strives for excellence in resident care in collaboration with all staff.
  • Regularly attends required meetings and in-services.
  • Participates in nursing committees and other activities, which promote the growth and development of nursing and/or resident care.
  • Serves as an ambassador and public relations representative to guests and other off-campus visitors.  Perform tours as required in the absence of designated personnel.
  • Uses tactful, diplomatic communication techniques in potentially sensitive or emotional situations.
  • Monitors and maintains all certifications required for Humility House Assisted Living staff and self. 
  • Holds bi-monthly nursing department meetings.
  • Other duties as assigned by Director of Assisted Living Services and Marketing.

REPORTING STRUCTURE:

  • Reports to the Director of Assisted Living Services and Marketing.

SUPERVISORY RESPONSIBILITIES:

  • Direct and indirect nursing care to the residents.
  • Oversees LPN and Resident Assistant personnel.
