Job Summary

The Occupational Therapist is responsible for evaluation and therapeutic implementation of illnesses and injuries which impede independent functioning of work, self-care, and play or leisure activities. Clinical responsibilities are such as outlined in the AOTA Scope of Practice.

Essential Duties and Responsibilities

1. Performs evaluations and treatments including but not limited to home evaluations, functional evaluation and cognitive evaluations as designed in scope of practice guidelines for OT.

2.  Performs patient assessments and plan and implements the plan of care including all necessary documentation and paperwork.

3. Performs evaluation for fitting of upper extremity: splints and braces and other adaptive equipment.

4. Recognizes the significance of changing patient condition and revises the plan of care based on ongoing assessment and evaluation.

5. Maintain and audits the medical record as appropriate to policy and procedure, regulatory bodies, payor sources and other appropriate sources.

6. Evaluates the effectiveness of the care given by OTA.

7. Adheres to the safety/infection control policies and procedures of the organization.

8. Identifies and adheres to regulatory guidelines such as JCAHO, Medicare, and/or Medicaid or other deemed regulatory bodies.

9. Maintains confidentiality and actively supports patient/family rights.

10. Attends interdisciplinary team conferences.

11. Keeps and turns in timely, accurate documentation and time activity payroll sheets.

12. Performs other duties as assigned.

Qualifications - Minimum


Must have current license for OT in the State of Ohio. 

Bachelor's degree (B.A.) from four-year College or university; or one to two years related experience and/or training; or equivalent combination of education and experience. Previous experience with elderly population necessary; home health experience valuable.
